Required document


First degree studies

1) a printed questionnaire of the candidate, completed by completing the form available on the personal recruitment account in the IRK system, signed by the candidate;

2) an application for the issue of an electronic student ID, signed by the candidate, printed from the applicant's personal admission account

3) school-leaving certificate in the original translated in English or Polish language by the sworn translator

- „maturity” certificate translated into Polish by a sworn translator or certified by the Polish consul in office in the country where the document was issued in the case of studies carried out in Polish;

5) If you have a secondary school-leaving certificate, you must submit a document certifying the legality of the certificate (legalization or apostille) and the administrative decision on recognition of the secondary school-leaving certificate obtained as part of the Polish education system, if recognition of the secondary school-leaving certificate obtained abroad as equivalent to the Polish secondary school-leaving certificate does not take place under Article 93 par. 1-2 of the Education Act.

A) Matura certificates, which are recognized in Poland by law:

• "maturity" certificates obtained in education systems of the European Union countries, the European Free Trade Agreement - the European Economic Area and the Organization for Economic Cooperation and Development (OECD), ie secondary school certificates or diplomas, secondary school-leaving certificates or diplomas or other documents which they entitle to apply for admission to higher education in the country in whose education system they have been issued and have been issued by a school or educational institution recognized by the state in whose territory or in whose education system the school or institution operates. For countries:

• certificates covered by international agreements - Belarus, China, Ukraine

B) Recognition of education by the decision of the superintendent of education - other countries

6) Nursing (in English): document certifying knowledge of English, included in the list constituting Annex 2 to the Regulation of the Prime Minister of 16 December 2009 on the manner of conducting recruitment proceedings in the civil service (Dz.U.2009.2018.1695) or a document issued by a verification committee verifying English proficiency;

7) a document entitling the holder to stay on the territory of the Republic of Poland

8) confirmation of payment for an electronic student ID (PLN 22)

9) a diploma or certificate of the title of winner or finalist of the central level olympics - in the case of a candidate using the right to obtain the maximum number of points from the total recruitment, which is due to the fact that he has a laureate or finalist degree of the central level;

10) a diploma or certificate of the title of winner or winner or finalist of an international or nationwide competition - in the case of a candidate using the maximum number of points from the entire qualification procedure resulting from the winner or winner or finalist of an international or national competition (including organized by the university)

 

• proof of payment for Electronic Student Identity Card (ELS) in the amount of PLN 22 for a special bank account number 58 1090 1014 0000 0001 2344 5842.

Once being qualified to submit documents for studies, the candidates who do not have a residing address in the European Union at that moment will be asked to provide a mailing address within the EU. Optionally, they can appoint a representative for the case residing in the Republic of Poland or a proxy for correspondence in the territory of the Republic of Poland.

 

For second-cycle studies full-time

1) a printed questionnaire of the candidate, completed by completing the form available on the personal recruitment account in the IRK system, signed by the candidate;

2) an application for the issuance of an electronic student ID, signed by the candidate, printed from the applicant's personal admission account;

3) a document authorizing to study: an original or a copy of the diploma confirming the attainment of a bachelor's degree, an engineer's degree, an engineer's degree or an equivalent photocopy of the document certified by the university;

4) In the case of a diploma issued abroad, it is required to submit an additional document certifying the legality of the diploma (legalization or apostille) and certificate of nostrification of the diploma, if nostrification is required.

Diplomas that allow to continue education at a higher level, in which the higher education system operates, the university which issued the diploma, allows you to continue learning at the right level - at the second level studies in Poland. The legal basis for recognition of a diploma is determined by art. 326 para. 1 and 2 of the Act of 20 July 2018 - Law on Higher Education and Science (Dz.U.2018, item 1668, as amended)

Foreign diploma may also entitle you to take up studies at the second level studies in Poland on the basis of an international agreement, as mentioned in art. 327 para. 1 of the Act.

In a situation when a candidate for a second degree studies has a diploma that does not directly give him the opportunity to study higher in the country where he was issued and the candidate wants to take a second degree studies in Poland, he must simply perform the nostrification procedure under the regulation of the Minister of Science and Education Higher of September 28, 2018 on the nostrification of diplomas of graduation abroad and confirmation of graduation at a certain level (Dz.U.2018, item 1881)

5) photocopy of the diploma supplement, if issued (the original document to be presented when submitting documents);

6) certificate of Polish language proficiency at least at B2 level issued by the State Commission for the Certification of Polish as a Foreign Language or certificate confirming completion of at least B2 level of the preparatory course for starting to study in Polish in units designated by the minister responsible for higher education, i.e. those operating at the academic centers of the Polish language for foreigners or a certificate confirming the passing of the exam from the Polish language conducted centrally by the Faculty of Humanities - in the case where the studies are conducted in Polish and the candidate holds a diploma obtained abroad;

7) a document entitling to stay on the territory of the Republic of Poland;

8) confirmation of payment for an electronic student ID card (22zl)

9) in the fields of: journalism and social communication, communication of culture and religion: one photograph in accordance with the requirements for issuing identity cards.

10) translated into Polish language diploma with a supplement by a sworn translator or certified by the Polish consul in office in the country where the document was issued
